Reports To: Lead Counselor
Northpoint Recovery Holdings, LLC, established in 2009, is a growing, behavioral healthcare treatment platform treating adults with substance use disorder and co-occurring conditions under the Northpoint Recovery brand and adolescents with mental healthcare conditions under the Imagine by Northpoint brand. The Company serves patients with commercial health insurance and is an in-network model. Northpoint has grown from two facilities to fifteen today entirely on a de novo basis with plans to expand rapidly in both existing and new markets. Northpoint Loveland has an opening or a Case Manager II in our Loveland, CO outpatient facility.
POSITION SUMMARY: As a member of the Northpoint team, the Case Manager II is responsible for helping patients secure resources, community services, and work collaboratively with the multidisciplinary team to provide ongoing support and communication to Northpoint patients. In addition, the Case Manager III will co-facilitate groups, take on training responsibilities, environmental safety and cleanliness initiatives, inventory tracking, and case management services. The Case Manager II will work under the direction of the Lead Counselor, counseling staff, and multidisciplinary team to coordinate care for all patients within the PHP and IOP programs.
ESSENTIAL RESPONSIBILITIES AND DUTIES INCLUDE BUT ARE NOT LIMITED TO THE FOLLOWING:

  • Coordinate training for clinical support staff and incoming new hires
  • Assess patient and and environmental risk factors per TJC reporting requirements as assigned by the Lead Counselor
  • Oversee and maintain inventory and stock for programming needs
  • Monitor and process patient drug screens
  • Complete initial intake, and assist with orientation, for new patients
  • Drive Company vehicle for official and approved and business; ensure safe transport of patients
  • Interact routinely with patients, observe behaviors and communicate significant observations to clinical staff
  • Document information regarding patient care and behavior as required in the policies and procedures
  • Utilize crisis prevention intervention when exposed to volatile patients to maintain personal safety while also ensuring safety for the patients and other individuals on the unit
  • Participate in maintenance of a clean, orderly and safe environment, and report needs for repairs and maintenance to appropriate staff
  • Assist in admissions and discharges of patients as assigned
  • Assist nursing and counseling staff in responding to emergency situations
  • Conduct and assist with activity groups as assigned, and document timlely according to policies and procedures
  • Assist patients with scheduling community appointments
  • Provide facilitation and co-facilitation of approved group sessions
  • Create and assist in updating treatment plans
  • Create educational curriculum as directed
  • Provide resourcing for crisis intervention (safety plans, contacting support, risk assessments, etc)
  • Assist patients in maintaining attendance to the program
  • Administrative duties that support the clinical team
  • Strive to ensure that patient's care at various locations is connected, and there are no gaps in care or communication
  • Assist with the referral process when needed
  • Serve patients in person and in multiple care settings (such as virtually)
  • Adhere to all company policies and procedures
  • Maintain confidentiality in accordance with established policies and procedures and standards of care
  • Other clinically appropriate services and special projects as assigned QUALIFICATIONS/REQUIREMENTS FOR POSITION:
    • Bachelor's degree from an accredited university in relevant field preferred
    • Minimum of 1 year experience in a patient care setting
    • Must be at least 21 years of age
    • Must have a valid drivers' license and a safe driving record
    • Strong communication skills and able to manage multiple priorities concurrently; organizationally savvy
    • Must have knowledge of Northpoint's mission and a willingness to embrace and actively support the unique culture and core values of Northpoint, and our work
    • Personal recovery experience not required, however, knowledge of recovery support systems and 12-Step programs preferred
    • Developed interpersonal and engagement skills
    • Experience with a growing company with multiple entities and geographic locations preferred PREFERRED KNOWLEDGE AND SKILLS:

        • Seeking a relevant licensure such as a CAT, CAS, LAC, LPC, LPCC, LSW, LCSW or LMFT REQUIRED CERTIFICATES/LICENSES/REGISTRATIONS:
          • Must maintain current nationally accredited CPR/First Aid/AED certification
          • Must have a valid drivers' license and safe driving record SUPERVISORY RESPONSIBILITIES:

                • Completion of a tuberculin screening during first week of employment
                • Completion of a pre-employment drug screening, post-accident and upon reasonable suspicion of use
                • Completion of orientation and required paperwork prior to reporting to work
                • Demonstrated computer literacy
                • Completion of CPR/First Aid Training within first 90 days of employment
                • Attendance at all mandatory staff development and training
                • Completion of background and professional reference checks

                  • $24.00 to $26.00 per hour Professional Development:
