Home Health for a travel nursing job in Denver, Colorado. Job
Description & Requirements - Specialty: Home Health - Discipline:
RN - Start Date: 07/21/2025 - Duration: 13 weeks - 40 hours per
week - Shift: 8 hours, days - Employment Type: Travel - Delivers
skilled nursing care through in-home visits to support healing and
manage chronic conditions. - Performs initial OASIS assessments and
follow-up visits as scheduled or clinically indicated. - Educates
patients and families on disease progression and effective care
management. - Manages a daily caseload of 3–5 in-home visits, based
on patient needs and scheduling. - Efficiently applies HCHB EMR
experience to ensure accurate and timely documentation. Required:
/n•/t1 years of recent home or outpatient hospice RN experience
/n•/tProficient in IDG reporting and hospice admissions /n•/tHCHB
