Focus on quality care! BAYADA Home Health Care is currently seeking
an Outpatient (Part B) Physical Therapist, PT, for a full time
opportunity performingoutpatient visits in independent living,
assisted living, and/or memory care communities throughout the
Loveland, CO area. As a Home Health Outpatient Physical Therapist,
you will be an integral member of a multi-disciplinary health care
continuum team that provides outpatient services to clients
affording them the opportunity to age in place. - Coverage area:
Loveland, CO / Fort Collins, CO - No weekends required. No on-call

desire to grow. Responsibilities: - Make outpatient visits to
clients in designated senior living communities in the Loveland /
Fort Collins area. - As necessary or applicable, supervision of
Physical Therapy Assistants, in accordance with state licensure
requirements and professional standards. - Attends and contributes
to patient care, staffing conferences and other related meetings. -
Makes referrals to other disciplines in order to address the
comprehensive needs of the patient. - Perform diagnostic tests and
measurements, such as the mobility/range of joints, transfer
status, stability, patterns and appearance of ambulation, strength
and endurance of muscles, balance testing, and safety assessments.
- Develop and implement appropriate individualized care plans,
including manual therapeutic exercises, gait training, balance, and
other interventions. - Continually assess and revise the Physical
Therapy - PT plan of care, and participate in the multidisciplinary
care plan meetings including community staff, as appropriate. -
Educate and instruct clients, family members, or community staff,
in therapy services and activities necessary to promote the
client's health, safety, and independent living. - Accurately
document observations, interventions, and evaluations pertaining to
client care management and services provided, utilizing a
state-of-the-art touch pad tablet. Qualifications: - A current CO
Physical Therapist - PT license. - Outpatient experience a plus but
not required. - Graduation from a program approved by the American
Physical Therapy Association or the Committee on Allied Health
Education and Accreditation of the American Medical Association, as
indicated by school transcript or diploma. - Ability to work
independently and manage time effectively. - Dynamic and strong
interpersonal, organizational, and problem-solving skills. - Solid
computer skills: prior experience with electronic medical records

Additional Information: - Pay: Weekly, starting at $40-$45 per
hour, based on experience. - Mileage reimbursement As an
