Job Description:
Stellar Living, renowned for its commitment to high-quality
senior care across various states, is seeking a professional MDS
(Minimum Data Set) Coordinator for our skilled nursing facility.
The MDS Coordinator will be instrumental in assessing our residents
and ensuring comprehensive care plans that align with federal and
state regulations. This role requires a keen attention to detail,
in-depth knowledge of regulatory requirements, and a compassionate
approach to handling resident assessments. The successful candidate
will play a key role in not only maintaining but elevating our
standards of care, contributing significantly to our reputation as
a leader in senior living services. Conduct and coordinate the
development and completion of the resident assessment (MDS) in
accordance with current federal and state rules, regulations, and
guidelines that govern the resident assessment, including the
implementation of RAPs and triggering of the CAA. Collaborate with
interdisciplinary teams to create individualized care plans that
meet the needs of each resident. Ensure accuracy of assessments and
care plans with timely updates reflecting the conditions of
residents. Train and oversee staff involved in the assessment
process to ensure compliance with national standards and
guidelines. Review resident’s medical records and information to
ensure an accurate Minimum Data Set. Coordinate and facilitate care
plan conferences with residents, family members, and other
appropriate staff as necessary. Proficient in MDS 3.0. Strong
organizational and communication skills. Ability to interpret and
implement regulations and standards. Strong analytical and
problem-solving skills. Proficient with healthcare management
systems and electronic health records. Medical Insurance Dental
Insurance Vision Insurance 401k Valid nursing license. Certified
Resident Assessment Coordinator (RAC-CT) preferred.
