Job Description:
Stellar Living, known for its commitment to high-quality senior
living services, is seeking an experienced MDS Coordinator to join
our dedicated team. In this role, you will be instrumental in
assessing and facilitating care plans for residents across
independent living, assisted living, and memory care facilities.
Your expertise will ensure compliance with federal and state
regulations, enhancing the quality of life for our community
members through precise and compassionate care assessment. Conduct
comprehensive assessments of resident care needs using MDS (Minimum
Data Set) and coordinate personalized care plans. Facilitate and
lead the interdisciplinary care plan meetings to ensure a
collaborative approach to resident care. Track and input MDS
assessments and data in a timely and accurate manner to comply with
state and federal regulations. Collaborate with healthcare
providers to continually evaluate the needs and conditions of
residents, adjusting care plans as necessary. Educate and train
staff on MDS procedures and updates in regulations to ensure
facility-wide compliance and understanding. Monitor and ensure the
accuracy and completeness of the MDS assessments to maintain the
highest possible quality of care. In-depth knowledge of MDS
processes and healthcare regulations. Strong organizational and
leadership skills. Excellent communication and interpersonal
skills. Ability to multitask and manage time effectively.
Proficient in healthcare-related IT systems. Medical Insurance
Dental Insurance Vision Insurance 401k Valid RN license in the
state of employment. Certified MDS Coordinator (CMDC)
preferred.
