Job Description:
AccentCare, a leader in home health care services, is looking
for a compassionate and dedicated Social Worker with a Master’s
degree in Social Work (MSW) to join our Hospice team. This role
involves providing psychosocial support to patients and their
families, managing case loads effectively, and working
collaboratively with other healthcare professionals to enhance the
quality of life for our hospice patients. The ideal candidate will
be someone who believes in personalized, patient-centered care and
is committed to helping patients navigate their end-of-life journey
with dignity and respect. Assess the psychosocial status of
patients and families/caregivers related to the patient's terminal
illness and environment. Provide clinical social evaluations and
plan appropriate interventions based on evaluations. Carry out
social evaluations, including family dynamics, caregiver abilities,
communication patterns, high risks for suicide, neglect, or abuse
and plans intervention based on evaluation findings. Counsel
patients and families about end-of-life issues and safe, effective
coping mechanisms. Develop and maintain cooperative working
relationships with agencies and organizations interested in hospice
care. Participate in the development of the individualized plan of
care, involving the patient and family, and attends regularly
scheduled interdisciplinary group meetings to revise the plan
according to patient and family needs. Assists family and patient
in planning for funeral arrangements, financial, legal, and health
care decision responsibilities. Strong interpersonal communication
skills Expertise in crisis intervention Ability to work
independently with strong organizational skills Deep understanding
of compliance with state, federal, and local regulations about
social health records and practice Skills in providing patient and
family-centered care Medical Insurance Dental Insurance Vision
Insurance 401k Valid state Clinical Social Worker license.
Certified by the Academy of Certified Social Workers (ACSW)
preferred.
