team!Responsibilities:Team Select Home Care is looking for a Family
Navigator to join our Team Select family. The Family Navigator (FN)
will coordinate the referral, intake, and onboarding process in an
office of the Long-Term Home Health Division. They will report
directly to the Operations ManagerEnsure under the Operations
Manager that the needs of the agency, referral sources, and all
patients/families going through the intake process are met through
communication, customer service, and oversight.Work with the local
Operations and Clinical teams to coordinate efficient, appropriate
intake and start of care processes.Promote ongoing patient and
employee retention through customer service and communication.Help
with all recruitment functions for the office; proficient in iCims,
Docusign, and all other facets of recruitment role.Assist Manager
of Operations in achieving financial metric goals.Assist Community
Relations Representatives in achieving market sales goals including
in-office and out-of-office tasks and events.Coordinate
non-clinical functions of the referral, intake, and onboarding
process together with Operational and HR staff to ensure all agency
requirements are met.Work with HR staff to promote efficient hiring
processes for new team members.Maintain a current awareness of the
abilities and limitations of RN Case Management staff in
collaboration with Clinical Supervisors and Operations
Manager.Demonstrate knowledge and adherence to the procedures of
Team Select and holds staff accountable for carrying out the tasks
outlined in the manuals, state and federal regulations and Team
Select's Best Practices.Maintain relationships with clients and
employees and promotes harmonious interpersonal relationships.Meet
all deadlines on special projects as assigned.Observe
confidentiality of all employee, patient/client PHI and company
issues.Follow and promote the Compliance Plan and
procedures.Maintain operational caseload as needed.On-call
responsibility as needed by Manager of Operations.Attend in
services.Perform all other job tasks as
assigned.Qualifications:Bachelor's degree in
Business/Marketing/Communications/Provider Relations preferred1 yr
experience in Healthcare field, Home Health preferredComply with
accepted professional standards and practiceExcellent business
decorum and appearanceSales and leadership capabilitiesOur
